  1. We will need the original prescription from an NZ Doctor. But you can fax us a copy first to 03 208 1722.

  2. We need a daytime delivery address where you will be available to sign on delivery.

  3. We need a daytime contact telephone number (in case the Pharmacist needs to contact you).

  4. We need your cheque or credit card details.
